Overview

NEWS2 (National Early Warning Score 2) is the updated version of the original NEWS, published by the Royal College of Physicians in 2017. It provides a single standardised method for assessing acute illness severity in adult patients using seven routinely measured physiological parameters.

The score is designed to trigger an escalating clinical response at defined thresholds, from increased monitoring frequency for low scores to immediate emergency team involvement for high scores (โ‰ฅ 7). A key feature is the "single-parameter rule": a score of 3 in any one parameter automatically triggers a medium-risk response, even if the aggregate total is below 5.

NEWS2 introduced two changes from the 2012 original: a revised ACVPU consciousness scale that includes new confusion as a separate level, and a dual SpOโ‚‚ scale system for patients at risk of hypercapnic respiratory failure (e.g. COPD), whose target saturation is 88โ€“92% rather than โ‰ฅ 96%.
